Please join us to kick-off our new PHASE initiative for 2017-2019.

At this meeting, we will focus on the goals and strategies for improving cardiovascular health for our patients during the next three years.

As with our previous grant round, Kaiser Permanente will require quarterly quality measure reporting at the clinic level, and CHCN will offer incentives funds for site-level participation and meeting site-level quality goals.

We will base PHASE site level participation on attendance of clinical/operations dyads or other pertinent care team members at the kick-off and on the on-time quarterly submission of site level hypertension blood pressure control data.

In addition, CHCN will highlight some final achievements from the 2015-2016 PHASE program.

• Clinical and Operational PHASE leads
• Quality Improvement Leads
• Care team staff working on PHASE at the site level
• Anyone involed in improving cardiovascular health for their patients!
